As a reminder, Grow Therapy asks that your malpractice insurance policy to meet the following requirements:
1. Your insurance must be individual, and the Certificate of Insurance must have your Legal Name (not just your LLC) listed under the "Insured" section
2. Your insurance limits must be at least $1 million per claim and $3 million aggregate
  - Exceptions:
    
    NPs in CA and FL $100K/$300K
    
    NPs in IN $500k/$1.5M
3. The name on your policy must match your Legal Name.
4. Your insurance is not expiring in the next 90 days.

If you are insured by a group or your policy limits do not meet our requirements, please use this guide for guidance on purchasing a new policy.

Please note that to maintain your credentialing providers are required to maintain active Malpractice Insurance that meets our requirements above. If it isn’t renewed by the expiration date, we’ll need to remove your credentialing, cancel any scheduled appointments, and notify your clients so they can be matched with another provider
